In rheumatoid arthritis (RA), the central contribution of innate immunity to the pathogenesis is supported by the strong association of HLA-DRB1 shared epitope (SE) alleles with disease and the demonstration of a SE-specific innate immune response. In turn, innate immune responsiveness in patients with RA is mediated by activation of SE- specific T cells. However, the aberrance in innate immunity also leads to an imbalance in the cellular ratios of T-regulatory and Th17 T cells, contributing to the immunologic dysfunction of the RA synovium. This imbalance of T cells can be recapitulated in vitro by skewing dendritic cells (DC) for IL-23 over IL-12. In a recent study, we found that IFNgamma-producing monocytes are essential for the acquisition of Th17 potential in the absence of IL-12 or IFNgamma. Moreover, we identified a new Th17 subset in which IFNgamma is produced by T cells, without a requirement for IL-23 stimulation or FOXp3 expression. Thus, we hypothesize that the combination of a dysregulated innate immune response leads to a concurrent imbalance of T-regulatory and Th17 T cells. Therefore, we plan to: 1) delineate the differences in innate immune response between RA patients and healthy controls (HC) using SE-specific monocyte activation assays and SE-specific T cell stimulation; 2) determine the functional contribution of SE-specific T cells to the innate immune response; 3) examine whether the imbalanced T cell ratio in RA patients is due to the abnormal activity of both T-regulatory and Th17 T cells; 4) perform kinetic studies of the SE-specific immune response; and 5) determine whether and how the innate immune response is influenced by treatment in RA patients and whether this correlates with changes in T-regulatory and Th17 T cell functions. The results from the proposed study will be important for development of early therapeutic strategies in rheumatic diseases as well as in understanding and treating the aberrant innate immune response in other inflammatory diseases including atherosclerosis.
